1. Complete an Application for Oversize Vehicle Parking Permit indicating the specific dates that you will need to park your oversize vehicle where signs are posted on the street or at the entry point to the oversize vehicle restricted area (OVRA) prohibiting oversize vehicle parking between 2 and 6 a.m.
2. Attach photocopies of any two of the following proofs of Los Angeles residency containing your name and your residence address in Los Angeles:
   1. Driver's license
   2. Property Tax Bill or rental/lease agreement
   3. Cable TV bill dates within the past 90 days
   4. Telephone bill (landline only) dated within the past 90 days
   5. Gas bill dates within the past 90 days
   6. Water bill dated within the past 90 days
   7. Electric bill dated within the past 90 days
3. Attach a check made payable to City of Los Angeles for the number of permits requested at $10.00 per permit.
4. Returned Check Policy: A payment by check that is returned for non-sufficient funds will be assessed a $35 returned check fee. The fee still applies if the check clears for redeposit.
